What the evidence adds up to

Minimal deviation carcinoma of the cervix is very rare, representing about 1% of adenocarcinomas of the cervix and less than one-sixth of well-differentiated cervical carcinomas. In a 1983 series of 13 cases, the two patients who died of cervical adenocarcinoma did so 6 and 14 years after initial diagnosis and treatment. That series reported better survival than earlier reports, but the cell types were more varied and the carcinomas were identified at early stages. An association with independent ovarian neoplasms was found. A 2021 case report states that patients with minimal deviation adenocarcinoma have a lower survival rate than patients with conventional human papillomavirus-related cervical adenocarcinoma.

A 2021 case report describes a 60-year-old woman with stage IB2 minimal deviation adenocarcinoma and lymph node metastasis. The report notes that surgical resection is the preferred and most effective treatment, but outcomes are often unsatisfactory. It states there is no therapeutic consensus, so each case must be treated individually. A 2011 case report describes a young woman with locally advanced minimal deviation adenocarcinoma who was successfully treated with concomitant chemoradiation followed by radical surgery, achieving a complete pathological response. The same report notes the intrinsic chemotherapy resistance and frequent failure of radiotherapy approaches that have raised the need to investigate multimodal strategies.

Minimal Deviation Carcinoma (Adenoma Malignum) of the Cervix

AbstractMinimal deviation carcinoma of the cervix is very rare, representing about 1% of adenocarcinomas of the cervix and less than one-sixth of well-differentiated cervical carcinomas. Composed of distorted glands having an irregular outline and deeply positioned in the cervix, minimal deviation carcinoma often requires hysterectomy to produce a sufficiently deep margin to permit identification of invasion. The gross appearance of the cervix may be normal in early lesions. Minimal deviation carcinoma often grows slowly. The two patients who died of cervical adenocarcinoma did so 6 and 14 years after initial diagnosis and treatment. The 13 cases in this report differ from those previously reported in that the cell types were more varied (most cases in the literature are of a mucinous cell type), the carcinomas were identified at early stages, and the survival of patients was much better than in earlier reports. An association with independent ovarian neoplasms was found.

Minimal deviation adenocarcinoma with elevated CA19-9: A case report

AbstractBACKGROUND: Minimal deviation adenocarcinoma is a rare malignancy with a high rate of misdiagnosis and high aggressiveness, and its diagnosis relies on histopathology. Surgical resection is the preferred and most effective treatment, but the outcomes are often unsatisfactory. CASE SUMMARY: A 60-year-old perimenopausal woman was admitted to the hospital and found to have elevated CA19-9 on physical examination without abdominal pain or vaginal bleeding. Clinical examination and positron emission tomography/computed tomography examination were unremarkable, magnetic resonance imaging examination was suggestive of dominant cervical lesions, and methylation examination was suggestive of malignant lesions. Tissue samples were taken from the suspected cervical lesion, and the final pathologic diagnosis was minimal deviation adenocarcinoma. Based on the pathologic diagnosis of suspected minimal deviation adenocarcinoma, radical abdominal total hysterectomy, bilateral oophorectomy, and pelvic and para-aortic lymph node dissection were performed. The final histological report confirmed minimal deviation adenocarcinoma of the cervix, stage IB2, with lymph node metastasis. Minimal deviation adenocarcinoma is a tumor with aggressive clinical behavior. CONCLUSION: Patients with minimal deviation adenocarcinoma have a lower survival rate than patients with conventional human papillomavirus-related cervical adenocarcinoma. A precise preoperative pathologic diagnosis may reduce the mortality rate due to missed optimal treatment with multiple surgical interventions. To date, there is no therapeutic consensus; therefore, each case must be treated individually.

A Complete Pathological Response to Treatment in a Young Patient with Locally Advanced Minimal DeviationAdenocarcinoma of the Uterine Cervix Undergoing Chemoradiation followed by Radical Surgery

AbstractMinimal deviation adenocarcinoma (MDA) is a very rare variant of cervical adenocarcinoma, this pathological entity is composed of mucinous very-well-differentiated glands deeply invading cervical stroma, and often surrounded by a desmoplastic reaction. Despite its benign histological appearance, MDA is typically characterized by aggressive clinical behavior and by relevant difficulties in achieving a final diagnosis. Moreover, the intrinsic chemotherapy resistance, as well as the frequent failure of radiotherapy approaches has raised the need to investigate the efficacy of multimodal strategies for the treatment of MDA patients. Here, we report a case of locally advanced MDA of the uterine cervix in a very young woman, who was successfully treated with concomitant chemoradiation followed by radical surgery.
