What the evidence adds up to

Primary squamous-cell carcinoma of the cecum is an uncommon diagnosis, with only four cases cited in the literature before 1971 and a fifth reported then. A 2009 case describes a 34-year-old male with a well-differentiated, keratinizing squamous-cell carcinoma invading the bowel wall up to the subserosal fat space; he received six months of postoperative chemotherapy and was free of disease two years after resection. These tumours show no distinctive clinical or gross features to separate them from the usual adenocarcinoma, and diagnosis rests on microscopy, with keratin and prekeratin stains, intracellular bridges, and negative mucin stains as supportive evidence. Proposed origins include embryonic nests of undifferentiated epithelium, epidermoid implantation at operation, inflammation, radiation, or teratomatous derivation.

For adenocarcinoma of the cecum, a 2018 retrospective study of 162 patients treated with right hemicolectomy and chemotherapy, with 48 also receiving adjuvant radiation therapy, reported 5-year local control, disease-free survival, and overall survival rates of 72.7%, 57.2%, and 62.6% respectively. Multivariate analysis identified age over 55 years (HR 1.0), T4 stage (HR 6.8), node-positive disease (HR 4.2), and absence of adjuvant radiation therapy (HR 3.0) as independent negative prognostic factors for overall survival, leading the authors to conclude that adjuvant radiation significantly improves disease-free and overall survival. An earlier 1986 analysis of 106 patients followed a median of 25 months after hemicolectomy found a median overall survival of 73 months and median disease-free survival of 37 months, with 22 of 106 patients (21%) failing locally, and 7 of those 22 also having distal failure; that paper suggested a group might benefit from adjuvant radiotherapy but left the choice between total abdominal irradiation and locoregional radiotherapy undefined.

Adenosquamous carcinoma of the cecum is aggressive. A 2024 case report describes a 71-year-old woman with a history of breast cancer who presented with bowel obstruction and an ileocolic mass; right hemicolectomy revealed advanced metastatic disease, and she died very soon after the operation. A 2004 case in a 71-year-old man with no lymph node metastasis but infiltration into the subserosal layer was also reported, with only 19 cases documented in Japan at that time; the theory of squamous metaplasia of colonic adenocarcinoma is widely supported for its etiology. A 2007 case of an adenocarcinoid tumour in the cecum in a 64-year-old man showed lymph node metastases in three nodes containing only the carcinoid component, though the malignancy potential and prognosis of such tumours remain unclear.

What is still missing is prospective data: the 2018 radiation study is retrospective and from three tertiary centres, the 1986 analysis is older and smaller, and the rare histologies are limited to single case reports. No randomised trial has defined the optimal adjuvant regimen for cecal adenocarcinoma, the role of radiation in squamous or adenosquamous variants is unstudied, and patient stratification by molecular markers or histologic subtype is absent. Funding for a multicentre prospective registry or trial, and systematic collection of these rare tumours, would be required before any firm guidance could emerge.

AbstractSummary Primary squamous-cell carcinoma of the cecum is uncommon. Only four cases have been cited in the literature previously. The fifth case is the subject of this presentation. This tumor has no characteristics, clinical or gross, which would help to distinguish it from the usual adenocarcinoma. The diagnosis is microscopic, and can be aided by keratin and prekeratin stains. The presence of intracellular bridges and negative mucin stains are supportive evidence. These lesions may arise from embryonic nests of undifferentiated epithelium, epidermoid implantation at time of operation, inflammation, or radiation, or they may be teratomatous.

Primary squamous cell carcinoma of the cecum

AbstractPrimary squamous cell carcinoma (SSC) of cecum is a very rare malignant neoplasm. The first case was reported by Schmidtman in 1919. Here, a well-differentiated, keratinizing SSC of cecum in a 34-year-old male patient is being reported. The neoplasm originated from surface epithelium, and invaded the bowel wall up to subserosal fat space. No predisposing factor was detected. Clinical features were that of an adenocarcinoma of the colon. Postoperative chemotherapy was given for 6 months to eradicate any micrometastasis. The patient is free of disease 2 years after having had the tumor removed. The case is being presented because of its rare occurrence.

Prognostic significance of adjuvant radiation therapy in adenocarcinoma of the cecum

AbstractPURPOSE: Local recurrence is a common failure pattern in adenocarcinoma of the cecum. This study aimed to investigate the potential role of adjuvant radiation therapy on oncologic outcomes of patients with adenocarcinoma of the cecum. MATERIALS AND METHODS: This retrospective study was carried out at three large tertiary university hospitals. We analyzed the characteristics, prognostic factors, and survival of 162 patients with adenocarcinoma of the cecum that were treated and followed up between 2000 and 2013. All the patients had undergone a right hemicolectomy and received chemotherapy with (n = 48) or without (n = 114) adjuvant radiation therapy. RESULTS: The subjects were 65 females and 97 males with a median age of 56 years (range, 17 to 90 years) at diagnosis. The 5-year local control (LC), disease free survival (DFS), and overall survival (OS) rates were 72.7%, 57.2%, and 62.6% respectively. In a multivariate analysis, age, tumor stage, node stage, and adjuvant radiation therapy were determined to be independent prognostic factors. Age more than 55 years (hazard ratio [HR] = 1.0; 95% confidence interval [CI], 0.06-0.32; p = 0.003], T4 stage (HR = 6.8; 95% CI, 3.07-15.36; p < 0.001), node positive disease (HR = 4.2; 95% CI, 1.94-9.13; p < 0.001), and the absence of adjuvant radiation therapy (HR = 3.0; 95% CI, 1.39-6.46; p = 0.005) had a negative influence on OS. CONCLUSION: Adjuvant radiation therapy significantly improves DFS and OS in patients with adenocarcinoma of the cecum.

Adenocarcinoma of the cecum: Analysis of 106 cases

AbstractThe natural history of surgically treated adenocarcinoma of the cecum has not been well defined. This report is a retrospective study of 106 patients followed for a median time of 25 months after hemicolectomy. The overall median survival rate was 73 months and the median disease-free survival rate was 37 months; 22 of 106 patients failed locally (21%). In addition, 7 of these 22 patients had distal failure. Thus, there appears to exist a group of patients likely to benefit from adjuvant radiotherapy. Whether total abdominal irradiation or locoregional radiotherapy is the method of choice needs to be further defined.

A case of adenosquamous carcinoma of the cecum

AbstractA case of adenosquamous carcinoma of the cecum was reported. A 71-year-old man complaining of constipation and admitted to our hospital for further investigation, because cecal tumor was pointed out by his home doctor. Barium enema examination revealed elevated lesion with central small ulcer. Abdominal computed tomography showed the wall thickness of cecum. Endoscopic examination revealed reddish flat elevated lesion with central small ulcer. Histological findings of biopsy specimen showed moderately differntiated adenocarcinoma. Operation was performed under the diagnosis of cecal cancer. The resected specimen demonstrated a type 2 tumor measurering 2.2×2.5cm in size. Histological findings of resected specimen showed adenosquamous carcinoma with no lymphnode metastasis. It was advanced carcinoma infiltrating into the excessserosal layer. Adenosquamous carcinoma of the cecum is considered to be a relatively rare disease and only 19 cases have been reported in Japan. In etiology of adenosquamous carcinoma, the theory of squamous cell metaplasis of colonic adenocarcinoma has been widely supported.

A Case of an Adenocarcinoid Tumor in the Cecum

AbstractAn adenocarcinoid of the cecum is a tumor that has seldom been reported in the literature. Its has the histologic features of both an adenocarcinoma and a carcinoid tumor. However, the malignancy potential and the prognostic behavior are still unclear. We treated one patient with this unusual tumor. A 64-year-old man was admitted with a complaint of right lower quadrant abdominal pain for 2 months. On colonoscopy and histological examination, the patient was diagnosed as having a cecal adenocarcinoma. A radical right hemicolectomy was done. Postoperative histologic analysis revealed that the tumor was composed of well-differentiated adenocacinoma and carcinoid components. Lymph node metastases were found in the three lymph nodes with only the carcinoid component.
