What the evidence adds up to

No abstract in this set reports a drug trial or any pharmacological intervention for Asperger syndrome. The 2003 review notes that clinical research has produced mixed results that do not clearly discriminate Asperger syndrome from high-functioning autism, and that children with pervasive developmental disorder achieve better outcomes when diagnosis is made early and appropriate community supports can be marshalled. No drug, dose, response rate, or survival figure appears in any of the four abstracts.

The 2002 review states that outcome in Asperger syndrome is very variable: many individuals with the characteristic childhood pattern function well in adult age, though personality traits tend to be highly persistent; others have a very poor prognosis from the psychosocial adjustment point of view and cannot manage independently. The 2007 article lists employment issues, comorbid mental and physical health conditions, neurological issues, possible problems with the legal system, and mortality rates as areas of concern, but provides no numerical data on any of these.

The 2002 background factors abstract says the cause has not been definitely established, that a genetic contribution is clearly important in many cases, that some cases have been linked to brain damage or developmental abnormalities arising in foetal life or early years, and that psychosocial factors appear to contribute little to causing the syndrome but may be very important for outcome.

What is still missing from the literature represented here is any controlled trial of a drug for Asperger syndrome, any quantitative outcome measure (response rate, survival, or standardised symptom score), any patient stratification by biomarker or genetic subtype, and any funding for such trials. Asperger syndrome: an update

AbstractThis article provides an overview of the history and clinical features of Asperger syndrome, and considers guidelines for clinical assessment and treatment. A review of issues related to external validity is provided, which points out the limitations of current research, and lists several potentially beneficial areas of investigation into the nosologic status of the condition. It concludes with a discussion of the unequivocal need of individuals with severe social disabilities for comprehensive and adequate educational services and other treatments irrespective of the fact that the validity and the utility of this specific diagnostic concept is far from resolved.

Asperger syndrome: to be or not to be?

AbstractPURPOSE OF REVIEW: Asperger syndrome is a pervasive developmental disorder characterized by impairments in social interactions, such as nonverbal behaviors, failure to develop peer relationships, and lack of social reciprocity with restricted, repetitive and stereotyped patterns of interest or behavior. The diagnosis of Asperger syndrome is increasingly common, and it is timely to review its phenomenology and treatment. RECENT FINDINGS: As there is a growing public awareness and acceptance of Asperger syndrome, clinical research has produced mixed results that do not clearly discriminate Asperger syndrome from high-functioning autism. However, research does indicate that children with pervasive developmental disorder achieve better outcomes when diagnosis is made early and appropriate community supports can be marshaled. SUMMARY: The history and phenomenology of Asperger syndrome is briefly reviewed, and recent literature relating to assessment and treatment is highlighted.

Others function along a spectrum in between excellent and highly restricted outcomes.

AbstractIt has not yet been definitely established what causes Asperger syndrome. A genetic contribution is clearly important in many cases. Some cases have been linked to damage or developmental abnormalities in the brain arising either in foetal life or in the first years after birth. It is possible that some instances of the syndrome are attributable to the combination of genetic and brain damaging events. Psychosocial factors in themselves appear to contribute little in terms of causing the syndrome, but may be very important from the point of view of outcome.
