What the evidence adds up to
In a retrospective case note review of 163 children with chronic anterior uveitis diagnosed between 1982 and 1998, 34 patients (21%) developed at least one complication (cataract surgery, ocular hypertension treatment, or visual acuity worse than 6/24). The strongest predictor of complications was severe disease at onset (p = 0.001). Other risk factors included uveitis present at the first eye examination, non-oligoarticular juvenile idiopathic arthritis, late onset arthritis, and male sex in the standard screening cohort. Factors associated with remission (inactive uveitis off all topical treatment for more than six months) included membership of the standard screening cohort, onset after 1990, white race, mild disease at onset, and a long interval between arthritis and uveitis onset. The authors concluded that most children remit without visually disabling complications and that targeting aggressive immunosuppression at high-risk patients before complications develop might reduce the complication rate.
A prospective observational study of 117 patients with active uveitis presenting to an emergency eye care centre in Brazil found that anterior uveitis was the most common form (70.1%), followed by posterior uveitis (26.5%) and panuveitis (3.4%). The majority of patients were male (63.2%), with a mean age of 43.2 years. Almost all had unilateral disease (94.9%) and acute onset, with a mean symptom duration of 6.2 days. Ocular pain (76.9%), redness (59.8%), and visual blurring (46.2%) were the most frequent symptoms. An etiological diagnosis was made at the initial emergency evaluation in 38.5% of patients. Patients with anterior uveitis presented with better visual acuity than those with posterior uveitis.
A study testing the hypothesis that stressful life events trigger recurrence of idiopathic recurrent acute anterior uveitis compared 35 patients presenting with a recurrence to 29 controls with other ocular problems. Using the Holmes and Rahe scale and the Spielberger state-trait anxiety inventory, the patients with uveitis did not report a higher number of stressful life events or higher levels of anxiety in the month preceding symptom onset. Scores for both groups were comparable to published norms for healthy adults of the same age and sex. The authors concluded that stress was not shown to be involved in triggering recurrence in this study.
A 2009 review proposed that acute and recurrent anterior uveitis may be associated with activation of Toll-like receptors by pathogen-associated molecular patterns (such as lipopolysaccharide from gram-negative bacteria) and by damage-associated molecular patterns (such as heat shock proteins and S100 proteins) released from ocular tissue damage, which could perpetuate or reactivate inflammation. A 2021 thesis applied genome-wide association study, Mendelian randomisation, and cytokine proteomics to acute anterior uveitis, identifying multiple susceptibility loci, environmental risk factors, potential biomarkers, and a polygenic risk score. What remains missing is prospective validation of the polygenic risk score in diverse populations, randomised trials testing whether early aggressive immunosuppression in children identified as high-risk actually reduces complication rates without unacceptable toxicity, and a clearer understanding of which specific innate immune triggers are clinically relevant in human disease rather than only in animal models.

An evaluation of baseline risk factors predicting severity in juvenile idiopathic arthritis associated uveitis and other chronic anterior uveitis in early childhood
AbstractBACKGROUND/AIMS: The clinical course for childhood chronic anterior uveitis can vary from mild, self limiting disease to bilateral blindness. The purpose of this study was to identify those risk factors at onset that predict disease severity. METHODS: A retrospective case note review of all patients with painless anterior uveitis diagnosed from 1982 to 1998. Patients were divided into two cohorts based on route of referral, diagnosis, and compliance with treatment. The standard cohort consisted of only those diagnosed from routine screening of juvenile idiopathic arthritis. RESULTS: Complications-cataract surgery, ocular hypertension treatment, and visual acuity <6/24. Remission: inactive uveitis on no topical treatment for >6 months. Results-163 patients were included. 34 patients (21%) developed at least one complication. The most significant predictor of complications was severe disease at onset (p = 0.001). Other factors included uveitis at the first examination (p = 0.034), membership of the non-standard cohort (p = 0.0001), non-oligoarticular disease (p = 0.02), and late onset arthritis (p = 0.024). Male sex was associated with increased complications in the standard cohort (p = 0.001). Factors predisposing to remission included membership of the standard cohort (p = 0.003), onset after 1990 (p = 0.016), white race (p = 0.015), mild disease onset (p = 0.003), and a long gap between arthritis and uveitis onset (p = 0.015). CONCLUSIONS: It is possible to characterise the severity of those with childhood chronic anterior uveitis at the onset of disease. The majority of patients remit without visually disabling complications. It may be possible to reduce the complication rate by targeting aggressive immunosuppression on high risk patients before complications develop.

The role of PAMPs and DAMPs in the pathogenesis of acute and recurrent anterior uveitis
AbstractThe pathogenesis of acute and recurrent anterior uveitis is poorly understood. Here, recent evidence is presented for the hypothesis that this disease may be associated with activation of receptors of the innate immune system, such as Toll-like receptors, by pathogen associated molecule patterns, such as lipopolysaccharide, derived from triggering gram negative bacteria. Acute anterior uveitis results in ocular tissue damage and the release of endogenous molecules (damage associated molecular patterns), such as heat shock proteins and S100 proteins that can also activate Toll-like receptors and thus perpetuate or reactivate intraocular inflammation.

Anterior uveitis and its relation to stress
AbstractBACKGROUND/AIMS: Idiopathic recurrent acute anterior uveitis (RAAU) is a common reason for attendance at ophthalmic casualty departments. Patients who suffer with this condition may have multiple recurrent episodes which are often debilitating and necessitate time off work. If recurrences are identified early and treatment initiated rapidly, the inflammation can be minimised and settles quickly on topical treatment with a speedy functional recovery by the patient. It has been our clinical impression that some patients with RAAU relate recurrences of inflammation to psychological factors. Therefore stress could be a trigger factor for recurrence, and at such times, patients should be alert to the early signs of uveitis and seek prompt examination by an ophthalmologist. This study tests the hypothesis that stressful life events are associated with the onset of RAAU. METHODS: The number of stressful life events in patients presenting to the casualty department with RAAU in the month immediately preceding symptom onset was compared with a control group of patients with other ocular problems taken from the casualty department using the Holmes and Rahe scale and the Spielberger state-trait anxiety inventory. RESULTS: 35 patients with RAAU did not report a higher number of stressful life events or higher levels of anxiety compared with 29 controls in this study on both Holmes and Rahe scale and Spielberger state-trait anxiety inventory. Scores for both groups were comparable with those published for normal adults of the same age and sex. CONCLUSION: Stress was not shown in this study to be involved in triggering the recurrence of idiopathic acute anterior uveitis.

Clinical and epidemiological characteristics of patients with uveitis in an emergency eye care center in Brazil
AbstractPURPOSE: To analyze the clinical and epidemiological characteristics of patients with uveitis in an emergency eye care center. METHODS: We conducted a prospective, observational study of patients with active uveitis admitted between May 2012 and July 2012 to an emergency eye care center. RESULTS: The majority of patients were male (63.2%), with a mean age of 43.2 years; 66.2% patients were of mixed ethnicity, 22.5% were Caucasian, and 11.3% were black. Anterior uveitis was observed in 70.1% patients, posterior uveitis in 26.5%, and panuveitis in 3.4%; no patient was diagnosed with intermediate uveitis. All patients had a sudden and acute presentation. The most frequent symptoms were ocular pain (76.9%), redness (59.8%), and visual blurring (46.2%). The majority of patients had unilateral disease (94.9%) with a mean symptom duration of 6.2 days. Diffuse and anterior uveitis were associated with ocular pain (p<0.001). Scotomata and floaters were more frequent in patients with posterior uveitis (p=0.003 and p=0.016, respectively). Patients with anterior uveitis presented with better visual acuity (p=0.025). Granulomatous keratotic precipitates were more frequent in patients with posterior uveitis (p=0.038). An etiological diagnosis based on the evaluation at the emergency center was made in 45 patients (38.5%). CONCLUSIONS: Acute anterior uveitis was the most frequent form of uveitis. Initial patient evaluation provided sufficient information for deciding primary therapy and aided in arriving at an etiological diagnosis in a considerable number of patients.

Immunogenetics of acute anterior Uveitis and comparison to Ankylosing Spondylitis
AbstractThis thesis comprehensively applies genome-wide association study, Mendelian randomization analyses, and cytokine proteomics to investigate the genetic basis and immunopathogenic mechanisms of acute anterior uveitis (AAU). Multiple susceptibility loci, environmental risk factors, and potential biomarkers are identified, and a polygenic risk score for AAU developed. These findings provide novel insights into the immunogenetics in AAU, and contribute to clinical translational studies.

Anterior uveitis in children
AbstractUveitis can cause major visual disabilities in children. Anterior uveitis accounts for approximately one-third of all cases of uveitis. The most frequent associated condition is juvenile rheumatoid arthritis, and Masquerade syndrome should always be considered. Screening patients with juvenile rheumatoid arthritis for anterior uveitis is worthwhile. The first-line therapy for anterior uveitis is a steroid. The second-line therapy is an immuno- suppressive agent, among which methotrexate is the most commonly used. Cataract, as a complication of the disease or of steroid use, can be treated by surgery. How- ever, operating in an inflamed eye of a child requires extra care. Glaucoma is another complication of uveitis, and can be treated primarily with antiglaucoma eye drops; surgery may be considered. Band keratopathy is another complication that may be treated with surgery.
