What the evidence adds up to
Adenoid cystic carcinoma of the breast is a rare malignancy, accounting for 0.1% to 1.0% of all breast cancers. It is classified as low-grade and has a favourable prognosis, with a low propensity for recurrence and metastasis. Despite being triple-negative by histology, its behaviour is unlike other triple-negative breast carcinomas. Complete surgical excision alone is curative in most circumstances in early-stage disease. Four retrospective studies have concluded that postoperative radiation contributes to improved survival, but there are no prospective or randomised data guiding treatment algorithms. Breast-conserving treatment including postoperative radiotherapy appears equivalent to mastectomy alone with respect to survival.
Routine axillary lymph node staging is not recommended because lymph node metastases are extremely rare. The value of adjuvant systemic therapies is not established. Late relapses can occur, so long-term follow-up is mandatory. A case report of a 32-year-old woman and a case report of a 79-year-old woman both describe management with lumpectomy and adjuvant radiation therapy. Early detection relies on diagnostic mammography and breast ultrasound.
The evidence base for this disease remains limited to retrospective studies and case reports. Prospective randomised studies are missing, and no specific recommendations for therapeutic management exist. The role of radiotherapy, while supported by retrospective data, has not been tested in a randomised trial. Patient stratification beyond early-stage versus late-stage disease is absent, and no funding for a prospective trial is evident in the published literature.

Adenoid Cystic Carcinoma of the Breast and Adjuvant Radiation Therapy
AbstractOBJECTIVES: Adenoid cystic carcinoma (ACC) of the breast is a rare special histologic type of breast cancer. This tumor is classified as low-grade and has a favorable prognosis, with rare occurrence of metastasis and distinctive characteristics. Due to the uncommon nature of this tumor, there are no specific recommendations regarding the therapeutic management of this malignancy. The aim of this review was to provide an overview of breast ACC clinical characteristics with a special focus on radiotherapy management. METHODS: A literature review was conducted up to January 2024 through a PubMed search using different combinations of pertinent keywords (eg, Adenoid cystic carcinoma alone, Adenoid cystic carcinoma AND breast Cancer AND radiotherapy; Adenoid cystic carcinoma AND breast cancer AND radiation therapy). RESULTS: This review highlighted the lack of prospective and randomized data guiding treatment algorithms for ACC. Four retrospective studies explored the impact of postoperative radiation and concluded that postoperative radiation contributes to improved survival. CONCLUSIONS: Thus, a consideration for radiotherapy indications in ACC of the breast could be further elucidated through prospective randomized studies.

Adenoid cystic carcinoma of the breast–A case report and review of the literature
AbstractAdenoid cystic carcinoma (ACC) is a rare type of breast cancer, and predominant adenoid cystic carcinomas are low grade and triple-negative by histology. Unlike other triple-negative breast carcinomas, ACC of the breast generally have a low propensity for recurrence and metastasis, with complete surgical excision alone being curative in most circumstances in early-stage disease. Here, we presents a case study detailing the clinical course of a 32-year-old female patient diagnosed with ACC of the breast. We emphasized the efficacy of standardized treatment protocols and highlighted recent advancements in research pertaining to this uncommon malignancy.

This review explores the existing literature and presents the case of a 79-year-old woman diagnosed with adenoid cystic carcinoma of the left breast, managed with lumpectomy and adjuvant radiation therapy.
